Privacy complaint or inquiry
You may submit a privacy complaint or inquiry by filling out information in the space below. Please include the following details in your complaint or inquiry:
- Detailed description of the complaint
- Date and time of occurrence
- Individuals involved in the occurrence
- Other pertinent information
- Regular Mail
- Annonymous
eHealth Ontario will accept anonymous complaints or inquiries; however, complaints or inquiries must include the sender’s name and address, telephone number or email address in order for a response to be sent. Do not include identifiable personal information or personal health information when submitting a complaint or inquiry.

eHealth Ontario will acknowledge receipt of a complaint or inquiry within five business days of the receiving it. eHealth Ontario will send a response of the outcome of the review or investigation to the submitter 30 business days of the receipt of the complaint or inquiry. If there is a delay in sending the response, the individual will be notified by post of the expected, approximate time frame.
