The ConnectingOntario clinical data repository (CDR) supports patients by providing care teams with a central point of access to an up-to-date, integrated care record. The ConnectingOntario CDR input standard defines the messaging format, data content, and terminology for contributing clinical data to the CDR. It may also be used by health service providers to implement OntarioMD’s hospital report manager for organizations contributing to this system and the CDR.
The standard aligns with other Ontario specifications as well as pan-Canadian and international document standards and specifications.
Please note the ConnectingOntario CDR Input Standard version 2.5 is now available for supporting data contribution of the complex organizations.
The CDR will provide access to provincial acute and community care data, improving the timeliness of care decisions by reducing duplicate tests and procedures and better supporting care transition points.
Starting with clinician-identified priority data, the ConnectingOntario will leverage health care organizations and provincial ehealth repositories to provide access to the following data: acute (e.g. discharge summaries, consultation reports, emergency department reports, visits and encounters, community care access centres , diagnostic imaging, and lab results). In the future, providers will have access to digital imaging common services, Ontario Laboratories Information System (OLIS) and Digital Health Drug Repository (DHDR) through ConnectingOntario
Contributing health service providers will supply these electronically-captured data types that can be transmitted in an HL7*-compliant format.
*HL7 is the primary standard used to move information from one computer to another within and between health care facilities.
