Clinical Document Specification

One of our evolving projects – the clinical document specification – targets exchanging clinical documents.

It was created to facilitate and expedite the wider electronic availability and transfer of clinical documents between and among hospitals, physicians, Community Care Access Centres and other health care organizations in Ontario.  The standard aligns with other Ontario specifications such as e-Referral and discharge summary, as well as existing pan-Canadian and international document standards and specifications.

Scope

Development work was supported by engaging with a wide-array of clinicians, organization and agencies. ConnectingOntario delivery partners and a number of GTA LHINs– have used this standard to inform document requirements for their respective clinical data repositories. These include:

  • Assessments
  • Consult Reports
