For health care professionals
From its origins as ConnectingGTA, ConnectingOntario Greater Toronto Area continues to evolve as Central Ontario’s regional hub for electronic patient information. About 42,000 clinicians from this region now have access to electronic acute, Community Care Access Centre, and Ontario laboratory information system patient data that is helping to improve patients’ journeys through the health care system.
ConnectingOntario Greater Toronto Area is currently implementing the next wave of connecting health care providers through its delivery partner, Sunnybrook Health Sciences Centre. An additional ~50 organizations and their care providers will join those currently using the solution to provide improved patient care and greater efficiencies in service delivery while contributing to reduced health care costs through elimination of duplicate testing and procedures.
When fully implemented, ConnectingOntario will bring improved patient care to the 50% of the provincial population that resides in the Greater Toronto Area who receive care from the region’s 50,000+ clinicians.

The Sunnybrook GTA program team is currently working with a new group of ~50 organizations in the acute care, rehab/complex continuing care, mental health and addictions and community organization sectors to enable them to use the ConnectingOntario Viewer to provide improved patient care. Sunnybrook is working with these healthcare providers during the 2016/17 timeframe to complete their implementation.
Of this group of participating organizations, 18 acute care sites will begin to contribute data to the ConnectingOntario Greater Toronto Area Clinical Data Repository:
Efforts are underway in the GTA to further expand use of the solution among the 50+ long-term care, acute care and primary care healthcare providers who participated in the initial launch of ConnectingGTA. Through this strategy, new clinicians and more diverse user groups will be able to leverage ConnectingOntario to realize greater efficiencies in patient care and create improved patient experiences wherever they receive care in Ontario.
